Agreement to Terms

By accessing or using the services provided by ConnectedMinds SLP (“we,” “us,” or “our”), including our website and teletherapy services, you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

Services Provided

ConnectedMinds SLP provides speech-language pathology services through teletherapy (online video sessions). Our services include evaluations, therapy sessions, and consultations for individuals of all ages.

Eligibility

Our services are available to residents of New York State. By using our services, you represent that you are located in New York at the time of service delivery. For clients under 18 years of age, a parent or legal guardian must consent to services.

Professional Relationship

The information provided on our website is for general informational purposes only and does not constitute medical advice or establish a therapist-client relationship. A professional relationship is established only when you have completed intake paperwork, provided informed consent, and scheduled services.

Teletherapy Consent

By participating in teletherapy sessions, you acknowledge that you have read and agree to our Telehealth Informed Consent, which describes the benefits, risks, and limitations of teletherapy services.

Payment and Billing

• Payment is due at the time of service unless other arrangements have been made.
• We accept major credit cards and may work with select insurance plans.
• You are responsible for understanding your insurance benefits and any costs not covered by insurance.
• We will provide a Good Faith Estimate of costs for self-pay clients as required by law.

Cancellation Policy

We require at least 24 hours' notice for cancellations. Late cancellations (less than 24 hours) or no-shows may be subject to a fee equal to the full session rate. This fee cannot be billed to insurance. Repeated late cancellations or no-shows may result in termination of services.

Your Responsibilities

As a client, you agree to:

• Provide accurate and complete information about yourself or your child
• Attend scheduled sessions on time
• Actively participate in therapy and complete recommended home practice activities
• Maintain a private, quiet space for teletherapy sessions
• Have appropriate technology and internet connection for video sessions
• Notify us of any changes in contact information, insurance, or relevant health conditions

Technology Requirements

Teletherapy sessions require a device with a camera and microphone, stable internet connection, and a private space. You are responsible for ensuring you have the necessary technology. We are not responsible for missed sessions due to technology failures on your end.

Intellectual Property

All content on our website and therapy materials provided during sessions are the property of ConnectedMinds SLP and are protected by copyright laws. You may not reproduce, distribute, or create derivative works without our written permission.

Recording Policy

Sessions may not be recorded by clients without prior written consent. Any authorized recordings must be used solely for personal, non-commercial purposes related to therapy goals.

Limitation of Liability

To the fullest extent permitted by law, ConnectedMinds SLP shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of our services. Our total liability shall not exceed the amount paid by you for services in the twelve months preceding the claim.

Termination of Services

Either party may terminate services at any time. We reserve the right to terminate services if you violate these terms, fail to pay for services, or if we determine that continued services are not in your best interest. We will provide appropriate referrals when possible.

Governing Law

These Terms of Service shall be governed by and construed in accordance with the laws of the State of New York, without regard to conflict of law principles.

Changes to Terms

We may update these Terms of Service from time to time. We will notify you of significant changes by posting the new terms on our website. Your continued use of our services after changes are posted constitutes acceptance of the new terms.
